Job Title: Psychiatric Nurse (Part-Time)
This position is responsible for providing nursing services to pediatric, adolescent, adult and geriatric clients with mental illness, chemical dependency, and dual diagnosis or mental illness with developmental disabilities. As a Nurse Coach you will utilize clinical expertise to manage assigned Post-Acute Care patients’ transition from acute care setting to the home setting through telephonic outreach to provide education, coaching and care coordination to accomplish the goals of the Post-Acute Care Program.

Maintain and provide update to all medical records and administer all routine tests and examinations for patients and supervise efficient working of staff and coordinate with nursing director to develop all clinical services.
